How do I qualify to receive special shoes and insoles from the Medicare Therapeutic Shoe Bill?
- You must be diagnosed with type II Diabetes and be covered by Medicare or the equivalent.
- You must receive a prescription from the doctor that treats you for your diabetes.
What are the next steps?
- Schedule an appointment to see your physician who is managing your diabetic condition.
- Request your physician complete, sign and date the Standard Written Order and the Statement of Certifying Physician forms.
- Ask your physician for a copy of the medical or progress notes of your visit.
- Call us to schedule your initial assessment for your diabetic shoes and inserts.
